Visas for Residents of: Illinois, Indiana, Iowa, Nebraska, North Dakota, Michigan,
Minnesota, Missouri, South Dakota and Wisconsin.
1. PASSPORT: 2. BRAZILIAN APPLICATION FORM: Download 2 (two) Brazilian application form, complete fully and sign. 3. PHOTOGRAPHS: 2 (two) recent photographs, 2 in. x 2 in., passport type. 4. LETTER: A letter from the institution or governmental agency sponsoring the applicant's trip, explaining the precise purpose and description of the applicant's assignment in Brazil. Also, letter must include information about the financial responsibility for the applicant while in Brazil. The letter must be on the institution's or governmental agency's letterhead. The letter cannot be faxed, it must be the original copy. E-mail invitations will not be accepted. PAYMENTS: 5. A Visa Fee of $200.00 per passport is required by the Consulate of Brazil, U.S. Post Office money order only!; the Consulate does not accept personal checks or other forms of payment. Please make payable to: Consulate of Brazil, 401 N. Michigan Ave., Chicago, IL. 6. A Service Fee of $60.00 per passport is also required (Cash, company check or money order

60613. 7. You are required to cover the cost of returning your passport with the stamped visa.
Please include postage payment or provide a self-addressed, prepaid envelope for the
delivery service desired (we recommend FedEx or UPS). Please note: Returning your package via the U.S. Postal Service, including Express Mail, Priority Mail, Certified or Registered letter, or any other SASE for the U.S. Postal Service, is NOT acceptable. 8. Applicants who will stay in Brazil for more than 90 days must present a police record check or good report issued by the applicant's local police department. 9. Round-trip ticket/reservation. A copy of a round-trip ticket/reservation, electronic ticket/reservation or electronic booking, or a signed letter from an issuing carrier or travel agent, on its letterhead, with the name(s) of the passenger(s), flight number or vessel, carrier name and dates of arrival in and departure from Brazil. Airline employees must present a copy of their airline indentification and a letter from the airline, on its letterhead, with dates of arrival in and departure from Brazil. Please note: If your passport does not
require a Brazilian Visa stamp or you are unable to obtain a Visa--for any reason--your
passport will be returned to you and you will be charged the full service fee.
