Brazil Visa Service, located at 3950 N. Lake Shore Dr., 1820D, Chicago, IL 60613, will only accept documents to be legalizaed from the states of Illinois, Indiana, Iowa, Michigan, Minnesota, Missouri, Nebraska, N. Dakota, S. Dakota and Wisconsin. Requirements for Legalization of Documents 1. The signature on the document must be notarized and the signature of the Notary Public must be certified at the County Clerk's or Secretary of State's office. (This original certification document will be attached to the document being legalized by either government agency.) PHOTOCOPY OF NOTARY'S LICENSE IS NOT ACCEPTABLE. 2. Birth, Marriage and Death Certificates must be originals or certified copies. 3. Requirement No. 1 is waived in legalization of official document issued by municipality, county or state government (local authorities such as the Secretary of States, Judge of the Superior Court) as long as they are certified copies or have an original signature an stamp or seal. Record checks/police good conduct certificates should be issued on official stationary and should also have a signature and seal or stamp. 4. Consular fee per document is $20.00 (company check, money order from U.S. Post Office only, or cash--personal checks are not accepted. Please make payable to Consulate of Brazil. 5. Service fee of $40.00 (company check, money order or cash--personal checks are not accepted). Make service fee payable to Rita Richter and send to 3950 N. Lake Shore Dr., 1820D, Chicago, IL 60613. 6. You are required to cover the cost of returning your document(s). Please include a self-addressed paid envelope for the delivery service desired (FedEx and UPS are recommended).
