INSTRUCTIONS FOR BRAZILIAN TECHNICAL/WORK VISA - TYPE V FOR U.S. PASSPORT HOLDERS

Brazil Visa Service will only accept Visas for Residents of: Illinois, Indiana, Iowa, Nebraska, North Dakota, Michigan, Minnesota, Missouri, South Dakota and Wisconsin.

1. PASSPORT:

A valid and signed passport. Your passport must be at least 6 months valid from date of intended arrival in Brazil and having at least two blank pages left. Amendment pages 22, 23 and 24 are not visa pages. PLEASE NOTE: THE PASSPORT MUST BE SIGNED AND INCLUDED WITH THE APPLICATION. PLEASE INCLUDE IN YOUR PACKAGE A PHONE NUMBER AND E-MAIL ADDRESS WHERE YOU CAN BE REACHED.

Eligibility:

* Scientist, professor, technician or other professional who comes to work in Brazil under a working contract and who receives a salary in Brazil;

* Provision of services to the Brazilian government based on an international agreement, convention or contract to which Brazil is a party;

* Employment contract with a corporation or other legal entity based in Brazil;

* Technical assistance based on a working contract signed between a Brazilian and a foreign corporation or other legal entity;

* Short-term technical assistance without a working contract;

* Professional training without a working contract, immediately after the completion of vocational training or a university education;

* Employees of foreign companies admitted to work in Brazil as trainees or interns at a subsidiary or Brazilian branch of the foreign company, provided that they are paid exclusively outside of Brazil by that foreign company;

* Foreign instructors or professors for training in foreign language instruction;

* Crew members of foreign vessels traveling to Brazil to operate in waters under Brazilian jurisdiction, as required under a charter, or service or risk contract entered into with a Brazilian;

* Crew members of foreign fishing vessels leased by Brazilian companies;

* Crew members or other professionals who perform paid activities aboard cruise vessels along the Brazilian coast, in the Amazon River basin or in other inland water.

THE CONSULATE WILL ONLY ISSUE THIS VISA AFTER THE PREVIOUS APPROVAL BY THE MINISTRY OF LABOR, ALLOWING THE APPLICANT TO WORK IN BRAZIL.

2. BRAZILIAN VISA APPLICATION FORMS:

The process for applying for a Brazilian visa has changed and the appliction MUST be completed online. The proper forms to be completed electronically are available by clicking the following link: https://scedv.serpro.gov.br Complete the appropriate electronic form, print the application form found online, sign your name and attach the photograph.

3. PHOTOGRAPHS:

1 (one) recent photograph, 2 in. x 2 in., passport type.

4. DRIVER'S LICENSE:

Provide a photocopy of the front of your driver's license or state identification card showing your photo and address.

5. COPY OF MARRIAGE CERTIFICATE AND BIRTH CERTIFICATE FOR IMMEDIATE FAMILY MEMBERS.

6. SWORN STATEMENT WAIVER:

Print and complete the Applicant's Sworn Statement Waiver and include with your packet of visa application material.

PAYMENTS:

7. A Visa Fee of $270.00 per passport is required by the Consulate of Brazil, U.S. Post Office money order only!. Please make payable to: Consulate of Brazil, 401 N. Michigan Ave., Chicago, IL. Note: Brazil Visa Service can obtain the U.S. Post Office money order on behalf of a client for a $10 service fee.

8. A Service Fee of $70.00 per passport is also required (Cash, company check or money order -- personal checks are not accepted).  Please make Payable to: Rita Richter. Send payment and materials to: Rita Richter, 3950 N. Lake Shore Dr., Suite 1820D, Chicago, IL 60613.

9. You are required to cover the cost of returning your passport with the stamped visa. Please include postage payment or provide a self-addressed, prepaid envelope for the delivery service desired (we recommend FedEx or UPS). Please note: Returning your package via the U.S. Postal Service, including Express Mail, Priority Mail, Certified or Registered letter, or any other SASE for the U.S. Postal Service, is NOT acceptable.

10. Citizens bearing passports of the Country of _______________, also are required to present a photocopy of your Resident Alien card (Green Card)--both sides--or a copy of your valid U.S. visa and I-94.

11. Non-criminal record. Non-criminal record issued by the local police department for the applicant is required.

Please note: If your passport does not require a Brazilian Visa stamp or you are unable to obtain a Visa--for any reason--your passport will be returned to you and you will be charged the full service fee.
